64B5-7.009. Dental Dispensing Registration

64B5-7.009 Dental Dispensing Registration. (1) A dentist who dispenses medicinal drugs for human consumption for fee or remuneration of any kind, whether direct or indirect, must register with the Board of Dentistry by completing and submitting to the Board of Dentistry Form DH5123-MQA, Dental Dispensing Registration, which is incorporated by reference herein (5/2026) and can be obtained from http://flrules.org/Gateway/reference.asp?No=Ref-19743, or the Board of Dentistry, Department of Health, 4052 Bald Cypress Way, Bin #C-XX, Tallahassee, Florida 32399-3256 or at at http://floridasdentistry.gov/resources/. The Board shall notify the registrant by letter of any deficiencies in the aforementioned form within 30 days after the form is filed. The registrant shall rectify all deficiencies in the form within one year from the date of such letter or the form will be processed as an incomplete form and the application file will be closed. (2) The Board shall register only those applicants who have completed and submitted the application form and remitted the dispensing fee set forth in Rule 64B5-15.025, F.A.C.
