64B-9.005. Active Duty Military, Spouse of Active Duty Military or Veteran Licensure

Active duty military and veterans seeking licensure pursuant to Section 456.024(3), F.S., shall submit to the Department a completed Florida Department of Health Active Military or Veterans Licensure Application, Form DH5000-MQA (07/2016), incorporated by reference and available at https://www.flrules.org/Gateway/reference.asp?No=Ref-07724. Spouses of active duty military seeking licensure pursuant to Section 456.024(3), F.S., shall submit to the Department a completed Florida Department of Health Active Military Spouse Licensure Application, Form DH5015-MQA (07/2016), incorporated by reference and available at https://www.flrules.org/Gateway/reference.asp?No=Ref-07725.
