65C-15.032. Family Case Record

(1) A child-placing agency the provides services to children shall have on file a record of the child’s family. Files may be retained electronically. (2) The family case record must be available for the Department’s review and contain the following: (a) Demographic information, including address, birth dates, race, religion, family composition, and persons important to the child; (b) The social history, including any psychological or psychiatric reports and medical histories; (c) Strengths and needs of the family and the services required; (d) The agency’s assessment and initial case plan; (e) Signed agreements between the agency and family or legal guardian; (f) Summary of dates of contact and progress toward goals; (g) Permanency status; and, (h) Discharge summary.
