69O-136.005. Captive Insurers

(1) All domestic captive insurers filing an application for licensure, pursuant to chapter 628, Part V, F.S., shall submit the following forms, and otherwise comply with the requirements therein:
(a) Form OIR-C1-1000, “Application for Permit Captive Insurer,” incorporated by reference in Rule 69O-136.100, F.A.C.;
(b) Form OIR-C1-1416, “Uniform Certificate of Authority Application (UCAA) Lines of Insurance,” incorporated by reference in Rule 69O-136.100, F.A.C.;
(c) The biographical information package as defined in subsection 69O-136.100(3), F.A.C.;
(d) Form OIR-D0-896, “UCAA Proforma Financial Statements, Property and Casualty Insurance Company,” incorporated by reference in Rule 69O-136.100, F.A.C.;
(2) All foreign captive insurers filing an application to redomesticate to this state shall submit Form OIR-C1-1001, “Application to Redomesticate a Captive Insurer,” incorporated by reference in Rule 69O-136.100, F.A.C., and shall otherwise comply with the requirements therein.
All the forms listed above in this rule may be obtained from the Office’s website at floir.gov and shall be submitted electronically via the Office’s iApply system at floir.gov/iportal. Forms relating to specific kinds of insurance are to be submitted only by companies issuing policies relating to the kind of insurance specified on the form.
