69O-157.021. Certificate

A certificate issued pursuant to a group long-term care insurance policy, which policy is delivered or issued for delivery in this state, shall include: (1) A description of the principal benefits and coverage provided in the policy; (2) A statement of the principal exclusions, reductions, and limitations contained in the policy; (3) A statement that the description of principal benefits is a summary of the policy and that the group master policy should be consulted to determine governing contractual provisions; (4) Person insured; (5) Person to whom benefits are payable; (6) Group contract number; (7) Certificate number; (8) Effective date; and (9) Time certificate is effective.
