25-9.033. Standard Forms

(1) An exact copy of all standard forms (uniform contracts, agreements, riders, service applications, service extension agreement or other form), pertaining to subscribers’ or customers’ services, must be included in this section. (2) All such forms must be given a number so that reference can be made to them. (3) When any such form is changed, altered, or amended, the revised issue must be filed with the Commission. (4) In this section, the utility must provide a detailed index of its forms.
