HAR §17-2026-62. Applications

(a) A facility operator shall obtain an application from each individual or family applying for the program according to the criteria as indicated in section 17-2026-63. (b) A facility operator shall conduct a personal interview with the applicant. Failure of the applicant to appear at the personal interview without reasonable notice may be deemed a withdrawal of the application and the facility operator shall not be required to process the application any further.
