HAR §17-602.1-31. The hearing request

(a) When the
household directs its request for a hearing to the
branch, the request shall be forwarded immediately to
the hearing office. The information forwarded shall
include:
(1) Name and address of the household;
(2) Name and address of legal counsel or other
representative;
(3) Date of request;
(4) Category and case number; and
(5) A brief statement outlining the reasons for
the request.
