(5) Identify barriers that will interfere with
the enrollee’s participation in program
activities;
(6) Specify support services that will be
provided by the department to address the
removal of barriers;
(7) Specify tasks to be performed by the
enrollee; and
(8) Specify time frames and schedules for
accomplishing the tasks.
(b) An enrollee who enters into a barrier
reduction plan shall be required to comply with all
conditions set forth in the plan and shall be subject
to section 17-684.1-39.
(c) Final approval of the barrier reduction plan
