(b) Prior to the admission of a child to a facility, the provider shall obtain in writing from the child's parents or guardians the following information: (1) The child's full legal name, birth date, current address, and preferred names; (2) The names and addresses of the parents or guardians who are legally responsible for the child; (3) Telephone numbers or instructions as to how the parents or guardians may be reached during the hours the child is in the facility; (4) The name, address, and telephone number of person who shall assume responsibility for the child if for some reason the parent or guardian cannot be reached immediately in an emergency; (5) The names, addresses, and telephone numbers of persons authorized to take the child from the facility; and (6) Health information concerning the child, including any allergies as required by section 17-895.1-41.
