HAR §11-280.1-326. Permit renewals

(a) On application, a permit may be renewed for a term of five years. (b) A renewal fee in accordance with section 11-280.1-335 shall accompany each application for renewal of a permit. (c) An application for a renewal shall be received by the department at least one hundred eighty days prior to the expiration of the existing permit and shall be submitted on the "Application for Renewal of an Underground Storage Tank Permitll form prescribed by the director. An application shall be deemed complete only when: (1) All required and requested information, including the application form, plans, specifications, and other information required by this subchapter have been submitted in a timely fashion; 280.1-212 3 5 8 4 1 §11-280.1-329 (2) All fees have been paid as prescribed in section 11-280.1-335; and (3) The director determines that the application is complete. (b) The director shall approve, approve with conditions, or deny a complete application for a permit to install or operate an UST or tank system or a permit renewal, modification, or transfer, required under this chapter.
