HAR §11-504-47. Letters of accreditation

After
accreditation has been granted, a certificate of
accreditation shall be issued to the applicant and
shall contain the following:
(1)
The date of issuance;
(2)
The date of expiration;
(3)
The name and address of the training agency,
institution, firm, or person, and the name of
the training course;
(4)
A statement as to whether the accreditation
is complete or interim; and
(5)
The name, address, and phone number of the
state certifying authority.
