HAR §11-79-29. Reinstatement following license revocation

(a) A licensed dietitian whose license has been revoked may submit an application for reinstatement of licensure, accompanied with the licensure fees for reinstatement. (b) An application for reinstatement shall not be submitted until one year or more after the effective date of revocation. 79-13 27.95
