HAR §12-10-71. Annotation of documents
Cite as Haw. Code R. § 12-10-71
All documents, correspondence, or other
material filed with the director shall include as part of the heading:
(1) The injured employee's full name;
(2) Department's case number;
(3) Date of accident;
(4) Name of the employer in whose employ the injury occurred; and
(5)
If applicable, the name of the insurer or adjuster.
