HAR §12-12-26. The health care insurance contract

(a) Every health care contract of
insurers subject to the insurance laws of the State, including any amendment, endorsement, or
rider to a contract, which provides for benefits under section 393-7, HRS shall be
approved by the Hawaii State Insurance Commissioner prior to submission to the director
under section 12-12-16.
(b) Nothing in the statute or this chapter is intended to amend, modify or change

any policy form approval requirements prescribed in the State insurance laws. [Eff:
