HAR §12-12-43. Notice and proof of claim

(a) Any covered employee claiming benefits or
someone acting in behalf of the employee shall furnish a written claim to the health care

contractor pursuant to the health care contract.
(b) The claim shall include documents required by the health care contract. [Eff:
