HAR §12-26-5. Contents of complaint

Each complaint shall contain the following: (1) The full name, social security number, address, and telephone number (if any) of the complainant; (2) The full name, address, and telephone number (if any, and if known) of the respondent or respondents; 26-3 §12-26-5 (3) A plain and concise statement of the facts constituting the alleged unlawful practice; (4) The date or dates on which the alleged unlawful practice occurred; and (5) Other information as required by the department.
