HAR §11-156.1-13. Reporting by health care providers and health care facilities; manner of reporting

A health care provider shall report a reportable disease listed in Exhibit A to the department in the form and manner as prescribed by the department, which may include but is not limited to, telephone and - electronic submission; provided that Category 1 diseases listed in Exhibit A shall be reported by telephone. For the purposes of this section, "electronic submission" means the automated generation and transmission of case reports from an electronic health record or other form of secure data transfer using the internet as approved by the department.
