(a) A health care provider, health care facility, or laboratory director shall report to the department within twenty-four hours an outbreak or suspected outbreak of a communicable disease in the manner as prescribed by the department. (b) A health care provider, health care facility, or laboratory director shall report to the department immediately in the manner as prescribed by the department a communicable disease or dangerous disease of unusual or uncertain etiology.
