HAR §11-172-14. On-site survey

The department may conduct an on-site survey of an applicant for initial certification or renewal of certification. The applicant shall provide access to all records necessary to verify compliance with certification standards, and the department may conduct interviews with staff and any other individual with the applicant’s permission. (a) Unless the application for a certificate has been withdrawn or dismissed, the department shall make its decision on the application within the required time. The decision may be: (1) An approval; (2) A disapproval; or 172-7 (3) A determination of noncompliance pursuant to Section 11-172-16. (b) On the date that the department makes its decision,, it shall send the decision to the applicant by certified mail, return receipt requested, and delivered’ to addressee only. (c) If the department approves the certificate application the form of the certificate may be in the form of a letter of approval, and shall include the effective dates of the certificate.
