HAR §15-300-43. Contents of petition

{a) The petition shall state the following: (1) Name, address, telephone number of the petitioner and the petitioner's legal counsel, if any, which shall be updated by the petitioner at all times; (2) A brief and concise factual statement of the petitioner's claim; (3) The law or rule involved; (4) The names of all respondents or identities against whom the petition is brought; and (5) A brief statement of the relief sought by the petitioner. {b) If the petition is not in substantial compliance with subpart {a), the board may refuse to file the petition and may request the petitioner to submit `a amended petition in compliance thereto.
