HAR §11-177.1-25. Reinstatement of expired certificate

(a) If less than one year has passed since the expiration of a certificate, reinstatement may be requested. (b) The individual requesting reinstatement shall submit to the department: (1) An application for reinstatement; (2) The regular renewal fee; and (3) Documentation of the required department approved contact hours (if any) for the category of renewal.
