HAR §16-85-54. Certification application

(a) There shall be three classes of
emergency medical service personnel: Emergency Medical Technician (EMT),
Advanced Emergency Medical Technician (AEMT), and Paramedic.
(b)
An application for certification shall be made under oath on a form to
be provided by the board and shall require the applicant to provide:
(1)
For EMT certification:
(A)
The appropriate fees including the application fee which shall
not be refunded;
(B)
The applicant's full name;
(C)
The applicant’s date of birth;
(D)
Evidence of completion of an EMT program;
(E)
Evidence of completion of didactic training in cardiac
defibrillation and IV access from a State-approved EMT
program;
(F)
Evidence of completion of 96 hours of clinical training
experience obtained through a board-approved agency;
(G)
Evidence of passage of the National Registry of Emergency
Medical Technicians (NREMT) EMT examination;
(H)
Evidence of current certification as an EMT from the
NREMT;
(I)
Information regarding any conviction of any crime which has
not been annulled or expunged;
(J)
If applicable, evidence of any certifications held or once held
in other jurisdictions indicating the status of the certification
and documenting any disciplinary action; and
(K)
Any other information the board may require to investigate
the applicant's qualifications for certification.
(2)
For AEMT certification:
(A)
All items listed in subparagraphs (1) (A), (B), (C), (I), (J),
and (K);
(B)
Evidence of completion of an AEMT program;

(C)
Evidence of passage of the NREMT AEMT examination;
and
(D)
Evidence of current certification as an AEMT from the
NREMT.
(3)
For Paramedic certification:
(A)
All items listed in subparagraphs (1) (A), (B), (C), (I), (J),
and (K);
(B)
Evidence of completion of a Paramedic program accredited
by the Commission on Accreditation of Allied Health
Education Programs (CAAHEP);
(C)
Evidence of passage of the NREMT Paramedic examination;
and
(D)
Evidence of current certification as a Paramedic from the
NREMT.
(c)
An application for temporary certification shall be made under oath
on a form to be provided by the board and shall require the applicant to provide:
(1)
All items listed in subparagraphs (1) (A), (B), (C), and (K);
(2)
Evidence of completion of the appropriate training program within
the past twelve months from the date of application;
(3)
The name of the applicant's employer for whom the applicant will be
performing services; and
(4)
Application to take the examination within three months of
completion of the training program.
Temporary certification may be issued only once to an applicant and shall be valid
from the date of issuance of the temporary certification to the date of release of
results.
(d)
It shall be the applicant's responsibility to furnish any information
requested by the board. In the event of any change in the information provided, the
board shall be notified within thirty days of the change.
(e)
Every application shall be signed by the applicant and each reference
shall be signed by the person attesting to the applicant's education, experience, and

3. Chapter 16-85, Hawaii Administrative Rules, is amended by adding a new
section to read as follows:
