HAR §16-93-4. Application for license by endorsement

In addition to the
requirements of section 16-93-2(c), an applicant for license by endorsement shall
comply with the requirements of this section and shall submit the following:
(1)
Verification of the NBOME certificate; or the FLEX transcript of
grades; and
(2)
Verification of any other osteopathic licenses held in other
