Citizenship

An applicant shall be a citizen of the United States and submit a certified copy or original of one (1) of the following as verification of citizenship: (3-31-22) 01. Birth Certificate. A birth certificate issued by a city, county, or state; (3-31-22) 02. Passport. A current passport issued by the United States Government; (3-31-22) 03. Naturalization Certificate; (3-31-22) 04. Consular Report of Birth Abroad or Certification of Birth; or (3-31-22) 05. Certificate of Citizenship. (3-31-22)
