Applicant Requirements

01. Applicant Health Inspection. The Department may request a copy of an applicants’ most recen t state, federal or third-party health inspection, if applicable. Such inspection records will be kept on file for one (1 ) year. (3-15-22) 02. Applicant Licenses or Registrations.
