Severe Reaction To Any Immunization

01. Reporting Requirements. Each case or suspected case of a severe reaction to any immunizatio n must be reported to the Department or Health District within one (1) working day of identification. Each reported case of severe reaction to any immunization must be investigated t o confirm and document the circumstances relating to the reported reaction to the immunization. (3-17-22) 03.
