01. Application Submitted by Participant. The participant must submit an application form to the Department. An adult participant, a legal guardian, or a representative must sign the application form. (7-1-24) 02. Application Submitted Through SSA Low-Income Subsidy Data Transmission. For low- income subsidy applicants identified on the SSA data transmission, the protected Medicare Savings Program application date is the day they applied for the low-income subsidy. (7-1-24)
