Uniform Assessment Requirements

01. State Responsibility for Publicly Funded Residents. The Department will assess residents accessing services through a publicly funded program according to uniform criteria developed for that program. (3-28-23) 02. Provider Responsibility for Private-Pay Residents. The provider will develop, identify, assess, or direct a uniform needs assessment of each private-pay resident. Completed no later than fourteen (14) calendar days after admission; and (3-28-23) b. Reviewed when there is a change in condition, or every twelve (12) months, whichever occurs first. (3-28-23) 03. Core Elements. The assessment of a private-pay resident must be based on the following: (3-28-23) a. Identification and background information; (3-28-23) b. Medical diagnosis; (3-28-23) c. Medical and health needs; (3-28-23) d. Prescription medications including routes of administration, and any treatments or special diets, if applicable; (3-28-23) e. Historical and current behavior patterns; (3-28-23) f. Cognitive function; (3-28-23) g. Psychosocial and physical needs of the resident; (3-28-23) h. Functional status; and (3-28-23) i. Assessed level of care. (3-28-23) 04. Results of Assessment. The results of the assessment for both publicly funded and private-pay residents are used to evaluate the provider’s ability to meet the resident's needs, and to evaluate whether any special training, licenses, or certificates may be required to care for certain residents.
