01. Renewal Request. An agency must request renewal of its certificate no less than ninety (90) day s before the expiration date of the certificate, to ensure there is no lapse in certification. (3-17-22) 02. Expiration Without Timely Request for Renewal. Expiration of a certificate without a timel y request for renewal automatically rescinds the agency’s certificate to deliver services under these rules.
