01. Home Visiting Coverage. (7-1-26) a. Assessment for medical, educational, social, or other service needs; (7-1-26) b. Development and revision of a plan to address goals; (7-1-26) c. Medicaid will reimburse for targeted case management services for eligible incarcerated participants thirty (30) days prior to, and thirty (30) days after, their release into the community.
