01. Behavioral PHA. The participant must have their PCP determine eligibility for Behavioral PHA. The participant qualifies by meeting one (1) of the following: (7-1-26) a. For an adult, a body mass index (BMI) of thirty (30) or higher or eighteen and one-half (18 1/2) or IDAHO ADMINISTRATIVE CODE IDAPA 16.03.26 Department of Health & Welfare Medicaid Plan Benefits Section 192 Page 60 lower. (7-1-26) b. For a child, a body mass index (BMI) that falls in either the overweight or the underweight category as calculated using the Centers for Disease Control (CDC) Child and Teen BMI Calculator. (7-1-26) 02. Wellness PHA. A participant who is required to pay premiums for eligibility under state children’s health insurance program (SCHIP.) (7-1-26)
