Pcs: Eligibility

01. Level of Care. The Department conducts an assessment to determine whether a participant’s medical condition impairs their physical or mental functions or independence, and whether they can remain safely and effectively in their personal residence when supported by authorized PCS. (7-1-26) 02. Redetermination. Required annually to reauthorize PCS or to refer a participant to a NF. (7-1-26) 03. Significant Changes. An assessment can be requested due to changes in a participant’s needs at any time. (7-1-26)
