Certificate Not Transferable

The certificate is issued only to the agency named in the application, only for the period specified, only for th e location indicated in the application, and only to the owners or operators as expressed on the application submitted t o the Department. The certificate may not be transferred or assigned to any other person or entity. The certificate is nontransferable from one (1) location to another. (3-17-22)
