Prior to accepting an individual into continued care the following is required: (4-6-23) IDAHO ADMINISTRATIVE CODE IDAPA 16.04.18 Department of Health and Welfare Children’s Agencies & Residential Licensing Section 408 Page 31 01. Voluntary Agreement. A voluntary agreement to remain in the program signed by the person turning eighteen (18), or a copy of a court order authorizing continued placement after the individual's eighteenth birthday. (4-6-23) 02. Assessment for Others’ Safety. An assessment to assure that an individual does not jeopardize the health, safety, and well-being of the children in care of the organization. (4-6-23) 03. Additional Continued Care Plans. A plan that prohibits individuals from sharing a bedroom or other sleeping rooms with a child. (4-6-23) 04. Documentation of Care Prior to Eighteenth Birthday. Documentation verifying the individual was in the care of the organization prior to their eighteenth birthday. (4-6-23) 05. Documentation of Need for Continued Care.
