Application For Services

An application must be completed and signed by one (1) of the following individuals on behalf of the eligible chil d for emergency assistance to be given: (3-17-22) 01. A Parent or Parents. (3-17-22) 02. An Adult Relative. An adult relative may sign on behalf of the child, when the child is residin g with them and they are responsible for the child's care. (3-17-22) 151. -- 159.
