(RESERVED) Fees – Table (Non-Refundable) Athletic Trainer Licensure Fee - Not more than $240 Athletic Trainer Annual Renewal Fee - Not more than $160 Directing Physician Registration Fee - Not more than $50 Annual Renewal of Directing Physician Registration Fee - Not more than $25 Alternate Directing Physician Registration/Renewal Fee - $0 Provisional Licensure Fee - Not more than $80 Annual Renewal of Provisional License Fee - Not more than $40 Reinstatement Fee - Not more than $50 plus  unpaid renewal fees
