01. Form of Petition. Any person petitioning for a declaratory ruling must substantially follow this form. (3-31-22) 02. Contents of Petition. The petition shall: (3-31-22) a. Identify the petitioner and state the petitioner’s interest in the matter, (3-31-22) b. State the declaratory ruling that the petitioner seeks, and (3-31-22) c. Indicate the statute, order, rule, or other controlling law, and the factual allegations upon which the petitioner relies to support the petition.
