LaSalle Street, 13th Floor Chicago, Illinois 60603 (312) 814-2420 Illinois Department of Insurance JB PRITZKER Governor ANN GILLESPIE Director TO: All Pharmacy Benefit Managers Registered in Illinois FROM: Ann Gillespie, Director DATE: July 9, 2025 RE: Company Bulletin 2025-13 – Public Act 104-0027 Pharmacy Benefit Manager Fee Under Public Act 104-0027, each pharmacy benefit manager (PBM) is required to file with the Illinois Department of Insurance (“Department”) specific data as outlined under 215 ILCS 5/513b2 (f). Instructions for submission: By August 1, 2025, each PBM registered with the Department shall submit to the Department, utilizing the PBM Covered Individual Reporting Template, the name of each health benefit plan it administers and provide the number of covered individuals for each health benefit plan as of the date of submission. This Company Bulletin is published on the Department’s website under Companies > Company Bulletins: https://idoi.illinois.gov/companies/company-bulletins-page.html By September 1, 2025, each PBM must remit to the Department, via the PBM Fee Form, an amount equal to $15 multiplied by the PBM’s total number of covered individuals across all health benefit plans the PBM administers. You may access the reporting template and fee forms for submission on the Department’s website under Companies > Company Tax Forms, Deposits, Fees and Online ePay for Taxes and Invoices: Company Tax Forms, Deposits, Fees and Online ePay for Taxes and Invoices. An officer signature attesting to the reported exposure count is required. A PBM may choose to submit the report and total fee simultaneously by August 1, 2025, or separately by their respective deadlines. Submit the reporting template as described above due August 1, 2025, to Illinois Department of Insurance Tax Administration Unit, P.O. Box 7087, Springfield, Illinois 62791. Submit the fee form and payment as described above due September 1, 2025, payable to the Illinois Department of Insurance, Attn.: Tax Admin. Unit, P.O. Box 7087, Springfield, Illinois 62791.
