LaSalle Street, 13th Floor Chicago, Illinois 60603 (312) 814-2420 Illinois Department of Insurance JB PRITZKER Governor ANN GILLESPIE Director TO: All Pharmacy Benefit Managers Registered in Illinois FROM: Ann Gillespie, Director DATE: August 11, 2026 RE: Company Bulletin 2026-09 – Pharmacy Benefit Manager Fee Under the Prescription Drug and Affordability Act (the PDAA, Public Act 104-27), each pharmacy benefit manager (PBM) is required to file with the Illinois Department of Insurance (“Department”) an annual report, as outlined under 215 ILCS 5/513b1.1, and to remit an annual payment based on that report pursuant to 215 ILCS 5/513b2(f). The Illinois Department of Insurance (“Department”) is issuing this bulletin to provide instructions for submission of the required payment. This Company Bulletin replaces Company Bulletin 2025-131, which applied only to PDAA payments for 2025. For more information about the PDAA’s applicability to PBMs for certain types of health benefit plans, please review Company Bulletin 2025-20.2 Instructions for Payments: By September 1, 2026, each PBM registered with the Department shall submit to the Department an annual report using the templates provided by the Department. The PBM annual reporting templates include the file named “File 4 – Reporting Cover Sheet Revised.xlsx”, which includes the name of each health benefit plan the PBM administers and the number of Illinois residents who are covered individuals in each plan. By September 1, 2026, each PBM must also remit to the Department an amount equal to $15 multiplied by the PBM’s total number of Illinois resident covered individuals across all health benefit plans the PBM administers. PBMs should submit a printout of “File 4 – Reporting Cover Sheet Revised.xlsx” with the required payment, payable to the Illinois Department of Insurance, Attn.: Tax Admin. Unit, P.O.
