The format for the collection of this information was created in accordance with statute and posted to the IDOI website. Collection of this data is required for two years: Due on July 1, 2023 will be data collected for calendar year 2022. Due on July 1, 2024 will be data collected for calendar year 2023. You may access the reporting template for submission on IDOI’s website under Companies, Bulletins, Company Bulletin 2021-12-template. Note that issuers are required to utilize this format when completing and submitting the respective date. Instructions for Submission: Submit the data as described above to DOI.MarketConduct@illinois.gov . These requirements are pursuant to the Department’s authority under the Illinois Insurance Code, specifically 215 ILCS 5/143.10d, 215 ILCS 5/132 and 215 ILCS 5/401 to receive and examine the nonfinancial business practices, performance, and operations of any insurance company and to administer the insurance laws of Illinois. Confidentiality of the underlying data filed with the Department is preserved under 215 ILCS 5/132.5(f).
