Section
125.40 Form of Application
An
applicant
seeking an economic incentive under
this Act shall submit a detailed application to the Department.

The application will be provided by the Department. The
application shall contain:
a)
the location of the project;
b)
the
amount of the capital investment the applicant will make in the project;
c)
the
number of new jobs that will be created as a result of the project;
d)
the
number of jobs retained by an existing applicant; and
e)
the
average salary of the jobs to be created or retained.
