Requirements as to Proper Form

Section 135 Section 135.2140  Requirements as to Proper Form Any document filed with the Securities Department pursuant to the Act shall be prepared in accordance with the form, if any, prescribed by the Securities Department.  Any such document shall be deemed to be filed on the proper form unless objection to the form is made by the Securities Department.
