Required Signatures

Section 815 Section 815.204  Required Signatures a)         All initial facility reports shall contain the name, address, and telephone number of a duly authorized agent to whom all inquiries and correspondence shall be addressed. b)         All initial facility reports shall be signed by a duly authorized agent and shall be accompanied by evidence of authority to sign the report and shall be notarized.
