Total............ b)         In furnishing the information required by subsection (a), the following shall apply: 1)         With respect to capital stock, indicate in the table or in a footnote the total number of shares to be authorized, the par or stated value of such shares, and the number of shares to be sold as part of the conversion. 2)         With respect to the funds to be received by the applicant from the sale of its capital stock, indicate in the table the estimated total amount of funds to be obtained, and in a footnote, state the price per share used in making the estimate.  The total amount and price per share shall be clearly identified as being estimates. 3)         With respect to Column A, the applicant should use the most recent balance sheet date required by Section 1075.2450.
