Section 660 Section 6.650  Form of Documents a)         Documents shall clearly show the file hearing number and the title of the proceedings in connection with which they are filed. b)        Except as otherwise provided, two copies of all documents shall be filed. c)         Documents shall be typewritten or reproduced from typewritten copy on letter size white paper. d)        Each document filed shall be signed by the party or by his authorized representative or attorney.
