Section 225.3000Â Resident
Assessment
a)Â Â Â Â Â Â Â Â The facility shall establish policies and procedures defining
the assessment process.
b)Â Â Â Â Â Â Â Â Prior to admission, each prospective resident shall receive a
complete physical examination with specific laboratory testing as follows:
1)Â Â Â Â Â Â Â Â Vitamin B12 and Folate levels;
2)Â Â Â Â Â Â Â Â Thyroid stimulating hormone level;
3)Â Â Â Â Â Â Â Â Complete blood count and blood parameters;
4)Â Â Â Â Â Â Â Â Electrocardiogram, 12 lead;
5)Â Â Â Â Â Â Â Â Chest radiologic study; and
6)Â Â Â Â Â Â Â Â Magnetic Resonance Imaging (MRI) study of the brain, when
possible.
c)Â Â Â Â Â Â Â Â Within two weeks after admission, each resident shall be
assessed by a neurologist, a registered nurse, a psychiatrist and a care
manager. A family member or other individual with knowledge about the resident
shall participate in the assessment.
d)Â Â Â Â Â Â Â Â The neurologist, with input from the registered nurse, shall
determine the resident's need for evaluation by a dietician or for other
specialized medical evaluations. The assessment shall be coordinated with the
pre-admission assessment to the extent possible to avoid duplicative testing.
e)Â Â Â Â Â Â Â Â The assessment shall be used, along with the pre-admission
assessment, to develop the resident's comprehensive care plan. At four-week
intervals, the resident's status shall be reviewed by the facility's nursing,
social service and activities and recreation staff.
f)Â Â Â Â Â Â Â Â In addition to the assessments required in subsections (c) and
(d) of this Section,
an Alzheimer's Disease Management Center shall conduct
and document an assessment of each resident every six months. The assessment
conducted shall include an evaluation of
the following:
1)
daily functioning
,
2)
cognitive status, and
3)
behavioral problems.

g)Â Â Â Â Â Â Â Â Assessment outcomes shall be reported to appropriate personnel
and to the resident and/or resident's representative.
