Section 280 Section 280.1030  Statement of Ownership a)         As a condition of issuance or renewal of a license to operate a hospice program, the applicant shall file a statement of ownership. b)         The licensee shall notify the Department of any change in the information required in the statement of ownership within 30 days before the change. c)         The statement of ownership shall include the following:  name, address, telephone number, occupation or business activity, and percent of direct or indirect financial interest of any person having a direct or indirect interest of five percent or more in the legal entity designated as the operator/licensee of the hospice program.
