Mobile Integrated Health Care Program (MIHP)

Integrated Health Care Program (MIHP)
An EMS MD shall submit to the Department a program plan
covering the following for the EMS System's Mobile Integrated Healthcare
Program (MIHP):
a)Â Â Â Â Â Â Â Â The
Department's Mobile Integrated Health application form provided by the Regional
EMS Coordinator;
b)Â Â Â Â Â Â Â Â Statement
from the Illinois licensed service provider that the provider has the resources
and personnel to meet both their response area and to support the MIHP;
c)Â Â Â Â Â Â Â Â Mobile
Integrated Health (MIH) System Policy;
d)Â Â Â Â Â Â Â Â MIH
Quality Improvement Plan to be submitted to the Department on a quarterly
basis;
e)Â Â Â Â Â Â Â Â MIH
Orientation and training plan;
f)Â Â Â Â Â Â Â Â MIH Medication
and Equipment list; and
g)Â Â Â Â Â Â Â Â List
of EMS personnel that participate in the program.
