Section 800 Section 800.980  Communicable Disease and Accident Reporting a)         Accident Reporting.  All deaths in a recreational area and those injuries occurring in a recreational area which receive a physician's care shall be reported to the Department of Public Health. b)         All outbreaks of disease in a recreational area which involve two or more persons shall be reported to the Department pursuant to the Department's Control of Communicable Diseases Code (77 Ill. Adm. Code 690). c)         For all the above accidents and illnesses, the Injury and Illness Report as provided by the Department is to be completed and returned to the regional office of the Department which serves the recreational area either by mailing or personally delivering it within 48 hours of the occurrence.  Appendix A contains the addresses of the regional offices.
